Dates in GREEN boxes are specific for *** SDV = Shallow-Depth Vaginoplasty or Zero-Depth Vaginoplasty ***. Please do not request one of these dates if you would like to have our standard SRS or Full Depth Vaginoplasty.

PLEASE READ BEFORE SUBMITTING REQUEST:

• We will post our availability from MAY to AUGUST 2027 for surgery with Dr. Bank and from AUGUST to SEPTEMBER 2026 for surgery with Dr. Prae on 8 May 2026 at 11.00 am in the morning, Thai time.
• Reservation requests will be accepted from 11:00 am on 8 May to 11:59 pm on 10 May 2026, Thai time. You have 3 days to submit your request at your convenience. Please note that this is not a first-come, first-served process.
• For clients whose client profile were submitted and approved before MAY 2025, your profile is no longer stored in our system. Please resubmit your client profile by 4 May 2026, Thai time. You may update your existing file and resend us OR fill-out a new client profile with your latest information. You can download a new client profile from https://supornclinic.com/surgery-protocols/
• For new clients, your client profile must be reviewed and approved by the medical team before a date can be reserved. If you have not yet submitted your client profile, please send it to us by 4 May 2026, Thai time. You can download the new client profile from https://supornclinic.com/surgery-protocols/
• You may request a surgery date with either Dr. Bank or with Dr. Prae. If you have no preference, you may say any surgeon. If no specific surgeon is requested, we will randomly assigned for you. Please note that switching surgeon is not allowed after the date is confirmed.
• You may inform us your preferred specific dates, a range of dates, or you may say any date in March (for example), or any date available. Please feel free to be as specific as necessary, and we will do our best to accommodate your preferences.
• Please use the same email address that you used when you sent us your client profile to reserve the date. Your record is tied to that email address. If you send us your request with a different email address, we may not recognize you. Please do not attempt to use multiple emails with multiple profiles that all belong to you; if found, your date will be deemed n your own volition.
• Please do not include your client profile in your email when requesting a date. If you wish to update your client profile, please do so before 4 May 2026, Thai time. Any requests sent with client profile in attachment will be forwarded to the medical team for approval instead of the reservation team.
• Please send only ONE email. If multiple emails are received, only the last email will be considered. Any emails sent before we update our availability will not be considered. – Please wait to send your email until you see BLUE & PURPLE boxes appears on all available dates.
• We reserve the right to give priority in date allocation to prospective clients who have never receive an offer or a provisionally offer from us. If you have received an offer in the past, either last-minute or regular offer, but did not reply, declined or previously cancelled a confirmed date, your request will be assigned second priority. If you do not intend to accept the offer, please do not apply.
• Our team will confirm receipt of your request by Wednesday, 13 May 2026, Thai time. If you have not heard from us by 6.00 pm, Thai time, on Wednesday, 13 May 2026, please contact us. 
• For successful clients, you will receive your confirmation from 13 May to 5 June 2026. We will send confirmations gradually after 13 May 2026. Please note that your provisionally reserved date is non-transferable.
• We reserve the right to amend the regulations, conditions, and details of our reservation process without advance notice. In the event of any disputes, we reserve the right of final interpretation and final decision.
